Followed the instructions, used source code for dreamweaver. Form posted to website no porblem, yet my tests are not going to the email specified. They are posting to my submission page, but I am not receiving them in my email specified in my account...any help here would be great. Thanks for your time.

Have you tried looking into the bulk and spam folders of your email?
I checked and tested the 2 notifications set for the form you mentioned
both of which are using the same email address.
The mail log showed that the email was sent:

You can also add JotForm to your email's contact list.
if it requires a specific email address use the following:
email@example.com and firstname.lastname@example.org
