Having trouble opening my saved forms

I keep making forms and saving them, However when I go back to my forms and try editing the form it says form is disabled this keeps happening to all my forms. Is there a reason why this is happening I would like to be able to edit my form even after the browser is closed and reopened.

Please look in to this and help asap.

Are you referring to the form 'Manager/Accounting completes' in your account cherzka? If so, we notice that the form is collecting Username and Password fields which is not allowed with JotForm. This is a violation of our terms of usage and so the account is suspended.

It is required to accept our terms of usage and agree to remove the guilty fields on the form in order to activate your account. Please confirm so that we can remove the fields and activate the account.
