    Also, upon finishing the test, the person receives a pdf-file with the results in his or her email, but in it all the answers for each option to the questions are listed: is there anyway we can change that?

    It is happening because the fields aren't hidden by default:

    Hiding the fields and then showing them conditionally (as you are doing currently) should solve your issue.
