Form on Weebly is distorted

    I created a form and it looks fine until I publish on Weebly.

    I have downloaded my form and on the URL I posted what is published. It is distorted. The form name is Elite Beach Villas Reservation Form.


    What can I do??

    It is recommended to use the JotForm app in Weebly to embed the form. If you want to embed the form using the code, please try using the iframe embed method and only the script within iframe tags.1626862945 60f7f561c9542 Screenshot 10
