    You are using the fixed layout form...you need to use a responsive layout form...here is one template you can use:http://www.jotform.com/?formID=40250461164141

    This appears to not be the case. I have tried using the "responsive" templates, and certain fields when added to a form to not "respond." These seem to be text area fields and also address fields.

    @leeqin1022

    Hi,

    Unfortunately, I could no longer access the website using the URL you provided http://www.sgmytaxi.com/jotform-testing/  so I am not able to test for issue.

    Please provide us another test page where we can do some test and check the problem.

    I checked your form http://www.jotform.me/form/40238228998466 also and can see conficting injected CSS code on it.

    Please try using this CSS only for the meantime while testing

    @media only screen

    and (min-device-width : 320px)

    and (max-device-width : 480px) {

    .form-textarea {

    width: 150px;

    }

    .form-header-group {

    font-size: 12px;

    width: 325px;

    }

    #input_37, #input_36 {

    width: 150px;

    }

    }

    Inform us of the result.

    Thanks!
