We are trying to reset our password to unlock our account. We are not getting the password reset email. How can we get this fixed?

Upon checking your account, it seems you do not have an account with us yet. Your current status with us is a Guest Account; this is why you are not able to reset your password at the moment. To set up your account and create your form, please sign up with us for a Free Account and create your forms. If you already created a form and would like to have this form under your account, please send us the form URL and we could transfer your form to your account.

We have an account already. Our username is focusphysio. We aren't sure where the emails are going when we select reset password.

Thank you for providing additional information to us. I was able to find your account and it seems the account was locked out due to multiple login failures for HIPAA-enabled account. I just reactivated the account on my end, please kindly try to login again and let us know if you face any issue again.
