We are not suspicious of the form.
The “informed consent” page does not render completely so when we tick on the consent box, the next page does not appear.
The form URL is Risk Assessment Office of the Attorney General (jotform.com)
We use EDGE browser
· How it shouldn’t look but does most of the time. If the page does not appear like this, clicking on next will not bring users to next page.

I accessed the form using Edge as browser but I was unable to replicate the issue. See demo below.
Can you please try to use other browsers?

We have tried IE11.
Our browsers are inside an office infrastructure governed by group policies.
Are there any exceptions we might try to allow page to render properly?
Are there any known issues in your Knowledge Base which would help us remedy this issue?

The group policies could be a factor. But just to confirm, can you please try to access the form using a different device in a different network outside your office?

I suggest you to request your IT team to remove whatever restrictions they have. They may whitelist JotForm's IP and Domains. Check the link below to view the complete list.
