Refund annual membership fee

    Please refund the annual membership fee charged for an account under my previous work email ( I was unexpectedly laid off from the company and my personal credit card was charged for the account. Once the refund is issued, please remove the credit card on file and close the account. I do not wish to continue with a jotform account. I am unable to log in to make these changes as I can't remember my password and password reset emails will be sent to an email address I can no longer access. Thank you.
