    I have tried different browsers and computers, the results are the same as you can see in uploaded screenshot.

    No URL field visible on the lightbox pop out...

    I've checked it on my side with Chrome and IE browser, and I can see a textbox for the URL like on the screen below.

    Please logout from your Jotform account, clear your browsers's cache and check it again. This might be of help.
