Multiple Recipients based on field responses

    Hello,

    I have a form

    http://form.jotformpro.com/form/40156809969973

    I would like the email addresses in the requestor, case manager, and SPED Dept  Chair email fields to all receive a copy of the submitted form as an attached pdf or doc.

    Please help.

    The easiest way to do that would be to create a New Email Notification directed to them with the Email Recipient Address set up to point to their emails. For the Email's Notification you would enable the " PDF LINK " ( {pdf-link} ) inside it by adding it to your email template with a button in the Form Field Box on the right.

    Sorry, Forgot to mention that for the Email Notification you will want to use a Multiple Recipient Email Setup.

    This doesn't help me I don't think because the notification asks me to identify a specified EMail Recipient.  I need the email addresses in the three specific email fields (requestor, case manager, and SPED Dept  Chair) of my to all receive the notification.  These fields may be different for each form submitted so a standard recipient is not possible.

    Hello,

    In this case, you need to create three different Autoresponder emails (looks like you've already started) for the requestor, case manager, and SPED Dept Chair.

    You can then select the fields as shown below:
