I'm unable to include submissions as PDF in the email notification

Upon checking, your Email Notification's PDF Attachment option is turned off.

Try turning it on, select a template, and don't forget to save the changes.

After adding my email address to the Notification email recipients on your form, I made a test submission and I regularly received the PDF attached to the Notification email:

I've also cleared your form cache.
