Login Failed After Multiple Password Changes

Hello I have changed passwords twice in the past three days. The error message now says Login Failed - Autoresponded and I'm not sure why. I copy and pasted the same password I used on the last change.

I am also concerned about the form not displaying on the page it is currently embedded on.

Please Help!
