Questions get cut off in email notifications

    In the email message that is sent when someone submits the online form, two of the questions in the email are incomplete. One of them just says "have you" rather than "Have you had continuous coverage for at least 12 months". The other says "If not" instead of "If not, why?". Can we complete the questions in these emails they are receiving from the form?

    I hope my question makes sense.  When should I expect a response to my question?

    Thank you for contacting us.

    I took a copy of your form and sent a test submission, and the complete questions can be viewed in full.

    I was not able to see what you mean, your email notification seems OK  from our end - Could you do a screenshot and upload it to your response?
