populate the email address and phone number based upon selecting the person's name from a dropdown list

    1) If I know the person's email address and phone number, is there a way I can populate the email address and phone number based upon selecting the person's name from a dropdown list?

    2) Do you mind looking at my form R.R. Intake Form, I can't seem to get the PDF to send to me and person completing the form. It is important for us to make sure both folks receive the full content of the form, even the bulleted paragraphs in the form, not just the fields where input has been entered. I look at your help video and THINK I did what it says. The difference is I'm selecting NEW FILE to be included and your video shows DEFAULT FILE to be included. Thank you.

    We do have a spreadsheet to form widget in which you can populate the field from an external spreadsheet data.
