I used your help.
1) Could not FIND "insert a free html field under Field Tools
Did manage to open a field, then found edit html
I pasted the NOT FOUND box of information into this field and hit submit.
You can embed your pdf document in form. This guide should help you: https://www.jotform.com/help/153-How-to-Display-a-PDF-Document-On-Your-Form
Please be informed that "Free text HTML" field is now renamed to "Text" and moved under "Form Tools" tab. Please check the screenshot below:
