People are getting this error: "widgets.jotform.io’s server IP address could not be found" for embedded form.

1. From a PT - I would like to register as a new patient, but am unable to complete the online form. Using the Chrome browser, I get the following error message when trying to enter information:widgets.jotform.io’s server IP address could not be found. The Safari browser does not work either.

Mostly people are getting this error here:
