My form is malfunctioning!!!

    My form is malfunctioning!!! I just received a submission with old questions and they did not answer new questions and required questions are NOT in the submission AT ALL!!! Yes, I published the form and it is showing correctly in the link that I sent them. However, the submission is incomplete and missing questions and have questions that I removed a long time ago!!!!
