Forms not working, can't log in so

I tried to lodge a leave form today. There was an error.

Also reimbursements are not working.

Last week I couldn't update a form. When I cloned it, the form was what I wanted.

I can not log into our account either.

There was an issue with our form builder and our developers have fixed the issue.
