I've set up a form (http://form.jotform.us/form/40295364957162) with three conditions.
The first two conditions work fine: 1. Show "Let us know how we can get you started (select one):" and 2. Show: "Institution or Collection Name" if "Join VertNet," in previous question is selected.
The third condition, Show "Institution or Collection Name" does not work if Host a training workshop" is selected. Not sure why that is.
Testing Chrome, FF, and Safari. None enable this condition.
Suggestions are welcome (although I'd prefer not to add a second "Institution or Collection Name" question.

Your second and third conditions are conflicting between each other. In order to fix this, we can combine them into one condition, like this:
