My approval flow no longer seems to be working. It sends the approval email but the status automatically shows completed rather than pending, approved or denied.

You need to use the merge branch element as you cannot have two start points.

Hierarchy is:

Form > Merge Branch > Attach your two conditional branch to your Merge Branch.
