My form Membership Application has a PayPal item for $150.00. It doesn't show up in the Preview though and it isn't showing up on the form on my site. http://www.theppginc.com/the-100-project-application/

You are unable to see this because you have only one product item in your payment question. Single product item is not displayed on the form simply because there is no need to select or display the total amount of the product selection.
If you still want to show the single product in your form, you should add a dummy free product in your form and use the custom css to hide it from form. This will make more than one product in your payment question and it will be displayed in your form. I have cloned your form and did the changes so that your single product can be displayed in your form.

One More Question: Before I made the updates you suggested, when the purchaser went to make a payment, they had the option of NOT creating a PayPal Account. Now that the change was made, the purchaser is forced to use their existing account or open a new one.
How can I give the purchaser the ability to pay by credit card without a PayPal account?
