I would like to create a form, where the user can visit the page and view all existing contacts and also have the option to add a new contact to the list.
I've attached a basic example of what I'm trying to create and look forward to hearing back.
P.s. It asked me to put a link for a page, but one has not been created so disregard the filler link I used.

Adding contact can be done by submitting the form.
Once the form is submitted, it will be added on your report table so I would suggest redirecting back your form users to your form after submissions so they'll be able to see their entry.
