I have someone (Barbara Orr) that completed our consent form, when she hit submit it showed an error and then she couldn’t see the form anymore but it’s not showing on my end in submission the error was in the 400s but she can’t remember the specific number. Can you help me figure out a way to recover this form please.

Hello Jen - I assume this is the form: Consent Form, correct? If so, I was able to replicate an error when I tried submitting a cloned form:
And upon further testing, it looks like the issue is with the Single Choice fields you've added into the FILL IN THE BLANK field:
There are no labels.
Now, if I put labels on them, the submission worked. So I would suggest adding a label here. Or if there's a need to have it blank, please consider adding at least a hyphen "-" that will serve as the label.
Please try that and let us know if that worked for you too.

As for the recovery, unfortunately, since the submission is not successful, there is nothing to recover.
I would recommend implementing the workaround I provided in my previous response, test it on your end if it'll work for you. And if it did, please advise that user to re-submit the form.
