I have 2 templates searchable by fortune5 - they disappeared yesterday morning, the were missing again this morning and one of them was missing tonight.
What would cause the to keep disappearing like this?

If you were referring to the JotForm Template gallery template, the forms shared there were being screened also for their useablity and purpose.
It was probably removed because it did not meet the criteria for it to be in the Template gallery.
We appreciate your effort in sharing forms that will contribute and benefit to other users as well. Just make sure that the form you are sharing are at least unique or does not have same content to other existing forms already. It must have greater impact and benefits to whoever will be using your shared forms.

a form for our team to use then?
is a video of how we are using
it now, is there a better way so it
keep getting deleted?
