I'm sorry to be a pain about something like this but I've completely forgotten to details I used to create an account. The account was used to create a form for a website I made for someone. Because of this I have no idea which email address and username I've used. I've tried using the forgotten email form with every email address and username I can think of to no avail. If anything the email used should be 'firstname.lastname@example.org' but that doesn't work. Here is the link to the form I made 'http://form.jotform.com/jsform/11042507538' if you could at least tell me what username or email address is associated with this form that would be very helpful.

Have you created a Free account before? I have checked your email address and only found a guest form linked to this email address.
To create a free account, you will see something like this on the left side when you go to My Forms page:
Click the orange Create a Free Account button and go through the process of creating your account.
You can also find the long orange button just below the login/password fields:
Note: Make sure to use the email address that you have used in your form so that the form you have created will be merged to your new free account.
