=?UTF-8?B??= <email@example.com> this has been the name for who has been sending me notification from Jotform, when and if a submission is received.
This has been happening for the last 24 hours.
Also, I have had reports from users that they have been stuck on the submit button--> Please wait..., for a long time and it never submits the form.
They have uploaded an acceptatble file with file size under the limit.
please provide feedback of any issues!

I made a test with a clone version of your form by sending a submission, but I was not able to reproduce the issue. It correctly shows this:
So, could you please let us know if this is happening with this form only? or does it happen with other forms too?
Also, let me tell you that I had no troubles uploading the a file nor to submit the form. It worked just fine.
So, please try cloning your form and test it. That cloned version must work correcly. Here is how to clone your form: Form cloning
If the cloned form work correctly use that to share it with your users, or if you have the form embeded to a website use remove the current code and use the code of the cloned form.
Please inform us if the issue still persists.

thank you for testing this. I have also done some testing and cleared the cache as suggested by other threads. I have not been able to reporduce the submit issue that a client spoke of. I believe this to be an issue with thier system. I have asked them again to submit information and to leave off the file upload. Just in case.
As for the emails that are sent to me upon submissions, it is still showing as: =?UTF-8?B??= <firstname.lastname@example.org>
I figure this is a glitchin your system that should be addressed.
