We have a shopify store and we require our customers to complete a form to collect certain passenger information.
The form has already been built on Jotform, but we want to add this to the Shopify Store, after the customer has purchased (so on the page directly after they enter the payment details)
Is there a way we can do this?

Hello Joe,
To get the embedding code for your form, we have this guide: Embedding a form
However, I did some checking and there doesn't seem to be a straight forward way to add it on Shopify after the checkout.
I would recommend contact Shopify's support to get advice on the best way to embed the form or to redirect to the form after checkout.

Sorry, I need this on the product page - I have attached a screenshot of a competitors website - I need it to offer the customer a drop down of dates, they select the date and they they are able to click checkout. Is this possible?

This can be possible by adding form calculation and dropdown element.

Thanks for this, however one the customer picks the date, they will checkout as normal through shopify - ie directed to the shopify checkout page and not a payment page through jotform.
