I noticed that you removed the Jotform logo in the notification mail template that has your client's Yahoo Mail account as the recipient. Please bear in mind that once you modify an email template, that email is then considered as "customized". As such, the Form Builder stops updating the email template automatically. Thus, with each changes you make to the form, you must update the notification mail manually.
In your form, I see something that's different from the notification mail
This is the reason why values for the bread field are not included in the notification mails sent to your client.
Solution to this is to delete then re-create the notification mail so that it contains the updated fields. If you are going to remove the Jotform logo again, just be sure to recreate the the notification everytime you make changes to the form.
