Upon checking your account, I did not find any issue with that. All your forms are there in your account. The form embed in your web page also loads fine. Please check the screenshot below:
It must be a temporary browser related cache issue. Have you already tried clearing your browser cache. Please do so and that should solve your problem. This guide should help you in clearing your browser cache: http://www.wikihow.com/Clear-Your-Browser's-Cache
