Can I put a password on my form?

    I actually have two questions. Can I put a password on my form?

     

    Also, I'm trying to embed my form on my website but it is not working at all and I've tried multiple times. My web host told me to look for a share button but I don't see one.

    Our froms do not have a password functionality. We do suggest to embed your form on a secured password section of your webpage. 

    Also, I'm trying to embed my form on my website but it is not working at all and I've tried multiple times. My web host told me to look for a share button but I don't see one.

    Here is your form's iFrame embed code:

    <iframe id="JotFormIFrame" onload="window.parent.scrollTo(0,0)" allowtransparency="true" src="https://secure.jotform.us/form/40331250585144" frameborder="0" style="width:100%; height:3501px; border:none;" scrolling="no"></iframe>

    By pasting/inserting this code to your webpage. This should display your form properly.

    It looks like I changed my form since this link was created. My new form should look like this
