    Hello mat1948,

Upon checking your web page, I found that you have used your form's source code to embed it in your web page. Please be noted that if you use your form's source code, any changes you make in form editor will not be reflected in your web page. That is the reason the dropdown option (Jasmine(Puncak Perdana)) is not getting displayed in your web page. It is part of the dropdown in your form editor but not part of the source code you have embedded in your web page.

You will have to get your form's source code again and replace it the existing form code you have in your web page. The following guide should help you grab your form's source code:  http://www.jotform.com/help/104-How-to-get-your-Form-Source-Code

    Thank you!
