It seems like my form is pretty unstable. Every other time I'm having problems opening the collapsed form. If I refresh the page it usually works and I am able to fill out the form but it's not really a solution as most users probably won't be able figure this out by themselves.
Please let me know if there's a solution to this problem

That's probably due to script conflict. Re-embedding your form using iframe embed code is the most efficient way to fixed this.
