My account is down, again.

    Sir-ma'am,

    My account has disappeared as has my surveys. My account email is info@10send.com and the account name is 10SEND.

    This same thing happened the other day for putting in a box for IDs. I have deleted that survey. I am really trying to comply with Jotform rules.

    Please explain why this is happpening and how I can fix this. I just don't understand.

    Your account has been suspended due to a form which contains a password field. Such forms are prohibited by JotForm's Terms of Use, as allowing them encourages phishing activities. due to this the phishing detector has disabled your account. We can enable to account back, but the form :
    40424053018139 has to be removed.
