submissions are not coming through to Jot form or via email

    When i test the form online - nothing has been showing up at jotform or in my email. It's like it is not registering at all. Can you help me with what I am doing wrong?

    Hello Julie,

    Upon checking your web page, I found that you have used your form's source code to embed it. Is there any specific reason to do so? I was not able to find any form related custom code in your web page.

    Upon taking a closer look at your web page, I found that you have two form tags in your form and both have different action URL

    <form method="post" action="/FaithFormation/Grade18/LentenFamilyActivityRegistration.aspx" id="Form" enctype="multipart/form-data">

    <form accept-charset="utf-8" action="" class="jotform-form" id="40424549819158" method="post" name="form_40424549819158">
    This is the reason when the form is submitted it is posted to the fist action URL and the data does not come though in your JotForm account.
    If you have not added any custom code in your form's source code, I would you to please use your form's iFrame embed code to embed it in your web page. The following guide should help you :
