I am able to see the issue to your form. Let me forward this thread to our developers so they can further check and investigate the cause of this.
While waiting for our updates, kindly clone the form by following this guide: How to clone an existing form from a URL. You should be able to see the changes when you preview or view the cloned form. Of course, use the cloned version instead of the original one.

Currently, there's an issue with jotform.me domain which the development team is working on right now to have it resolved. I think this is why you were not able to see the changes made on your form.
For the meantime, you can use http://www.jotform.pro/form/40452909611452
We'll keep you posted on this matter, and we apologize for this inconvenience.

The domain jotform.me is now fixed. You should be able to view and make submissions on your form again. There is no longer need for you to change domain names.
Please let us know if you have other questions.
Thanks for your patience and we apologize for the inconvenience this may have caused.

I'm sorry for the trouble caused to you.
I was able to replicate the problem you are having. I cloned your form in my account and when I compared with the form in your account, I was able to see the difference. There are several fields which is actually not visible in your form currently. Please take a look at the following form and see if this reflect all your changes: http://form.jotformpro.com/form/40472251085954?
I am creating a bug report and sending it to our back end team so that they can also have a look. We will get back to you as soon as we have any update on this.
While our back end team investigates this issue, I will suggest you to please clone this form to your account which will display all the fields you have added. The following guide should help you in form cloning: http://www.jotform.com/help/42-How-to-Clone-an-Existing-Form-from-a-URL
