Our sincere apologies apologies to the inconvenience that may have caused. I will now forward this issue to our higher ups for further investigation and for further assistance.
Can you please try to change your secure form url from https://secure to https://www to fix the issue as a temporarily workaround?
Your form code should be look like this:
<iframe id="JotFormIFrame" onload="window.parent.scrollTo(0,0)" allowtransparency="true" src="https://www.jotform.me/form/40418561877462" frameborder="0" style="width:100%; height:560px; border:none;" scrolling="no"></iframe>

Hi, as a temporary alternative option, you can also re-embed the form using jotform.pro domain.
Like this http://www.jotform.pro/form/40418561877462 . Please take note of the domain change in the URL.
Inform us if this does not work for you.
