I have created a form with conditional text to go with each question - when testing it & submitting to e-mail - the report comes via e-mail with only questions & answers & none of the condidtional text (which is feedback for each question).
Is there a way to get it e-mailed to me with all of the Header & conditional text to show as well - so I'll have a full report with questions, answers and feedback?

You can insert custom text onto your form's email notification template, but not conditional ones. Once you add custom text to a template, it will be always be displayed on the email alerts.
There is one workaround, and that is by replacing the conditional text with conditional textareas prefilled with the message and is set to "Read-only". Also, the form should be set to hide empty fields on emails.
Here's an example:

Let me look at the custom text onto form's e-mail notification first. How do you do that, Please?

You can easily add custom Text to the Notication body by simply pasting the text into it. Pleas see this gif file while adding the text to the Notification:
Please keep in mind that once you add custom text to notification content, it will always appear in the email you receive no matter it was conditional or not.

Hi - I followed your instructions & sent a test e-amail & it came through fine. I'm cloning the form & adding to FB & testing & now the e-mails all look back to default & I don't see the one I customized? Do you know what happened?

It is probably that Email Notification or updated form were not saved. Please make sure to use a Finish button to save your changes on Email Notifications. If you simply close the Email Notification window, it will not save your updates.

Are there 'glitches' where sometimes will work right & others not?
I have added red text to my e-mail forms (for the conditional logic part) & have received 4-5 entries this evening - all came through with the red text as it should - but the last entry did not.

No, such a glitch should not exist. Your email templates are static, and should therefore show the red text at all times.
I tested your form numerous but could not recreate the problem you have described. Can you please try again ang let us know if the issue is resolved or not?
