I updated a form for another individual and when I refresh our website, the revised version is not appearing. I have saved the changes. Is there something that I am supposed to do to have the revised form appear? (The originator of the form has left our company.)Page URL:

I can see that your are using your form's source code in your website always remember that when form source is embedded to your website whenever you made a revision to your form you need to update to form source from your website as well meaning that you will remove the old source that is embedded to your form then grab a new source code from Jotform.
I suggest you use the regular embed code since I can see that there is no other JS library in your webpage that will cause some problem. Here's how you can get your http://www.jotform.com/help/67-Which-Form-Embed-Code-Should-I-Use
