I have gone to the Account Settings page and tried to cancel the auto-renewal, but it takes me to a Plimus page and asks me for a username and password for that site. Unfortunately, I have never logged into that site.
So, how do I go about cancelling my subscription?

Your Plimus username is a long string of numbers from the original email that you have received when you paid for JotForm.
If you can't find your email that has the username/password, click on the link below and enter the email address that you used when you paid for your subscription. https://secure.plimus.com/jsp/forgot_password.jsp??designId=1
That will reset your password and you can then the details will be sent to your email.
Or you can simply ask us to cancel your subscription for you. Just confirm to us that you are willing to cancel your subscription and we will process it right away.
