JotForm: How to create Approval Workflow

Good day,

I would like to create an online leave form that will allow the form to be sent to the direct supervisor for approval and after approval the same link should come back as a submitted form to me as the form creator.

Is that possible?

I cloned your form to check. You can add Approval WorkFlow to your form.

1- Go to your Form Builder page. Click the arrow button 1 and then click Approvals button 2.

2-In the Approval Workflow window, you can add different conditions and edit EMAIL preferences. To change supervisor email, click the Settings button 1 and change email field 2.

3- You can customize both supervisor email, and customer email preferences by clicking the Settings button near the Email box 1.

4- In the Customize Email window, you can change email subject, content, recipients, and etc.
