I WOULD LIKE TO ASK THE CONTACT IF HE OR SHE WOULD LIKE ME TO CALL. HOW DO I INCLUDE THIS QUESTION ON THE THANK YOU PAGE? i WANT TO GIVE THE CONTACT THE OPTION TO ANSWER YES OR NO.

Thank you page is actually not a form so even if you add HTML fields there to take user input, the answer will not be captured or recorded in submissions.
Let me ask you some investigative questions based on which we can provide you an appropriate solution.
What happens when user selects YES or NO in the thank you page? Do you want to redirect to some other page?

My goal is to get their reaction to the video which will propose that they get a more in depth evaluation from me on the phone. I want to know how they react to this suggestion.

Probably the best way to achieve this result is that you create a form that will be your thank you page.
In this "thank you page" form you will embed the video using the text tool under form tools:
Edit the width and height values if you wish.
Then add a radio button with the desired question. You can take a look or clone the test form I created:
Click preview and copy the form url.
Now go back to the other form (the main one) and link the user to the thank you page form by using the "custom url" option on the thank you page wizard and pasting the url:

In this case you will receive 2 submissions, one of the main form, and other of the thank you page form with the yes or no answer on the contact request.
