HiPAA compliant survey

    Greetings,

    I see that your silver and gold levels are HiPAA compliant. Is there additional cost for that beyond the monthly fee?

    I have a couple of other general capability questions before signing up:

    1. I intend to create a simple online quiz with a few conditional questions. In other words, depending on the response to question number 2, they would get different versions of question number 3.
    2. I want the quiz to "embed" on another page so that it matches my branding and has features to help motivate respondents to complete the quiz. How does that work with JotForm? I will have some programming help. I use Kajabi and SamCart in my online offerings.
    3. A crucial data confidentiality question: The participants will answer the questions about their health situation. That means it will gather "personal health info". That info needs to be treated carefully. I want to export encoded info to Kajabi. In other words, I want their email, name, and a score of their responses, but not the details of the questions. So in other words, can Jotform "score" the questions and give that score to Zapier or directly to Kajabi?

    This is possible. You may create multiple conditions and then hide and show the fields.

    Currently, we don't have a direct integration with Kajabi yet, but you can use third-party apps such as Zapier.

    Thanks for your response.

    You clearly addressed questions 1 and 2 thank you.

    Question 3 wasn't about whether you have an integration. I'm asking there about what Jotform can do with the data before it is sent via Zapier or other means. I'm asking if Jotform can score a quiz and output a composite score, and how customizable is that process. And then that info can be exported. Can you speak to that, please?

    If you do not want to use the quiz form app, you may also use calculation values to identify correct answers and calculate the score using Form Calculation widget.
