I'm having issues with form fields in emails and PDFs

    For my form https://form.jotform.com/212538203084045 I am having issues with the email and PDF templates:

    1. In the email, I only want to display the patient's first name, not their whole name and title, how do I do this?
    2. In the PDF "Approval Med Cert", I don't want to include days (monday tuesday etc) in the date fields in this letter, how do I change this?
    3. in the same PDF, am trying to get the current date to display next to date of issue - what am I doing wrong?
    4. In the same PDF, the address doesn't display well - it includes Street name, suburb, etc not just the netered details when in the text box. When I insert the field, there is a big gap and it looks silly - how do I fix this so the address only displays and not the field labels?
    5. In the text, date from and to also include day names, how do I avoid this?

    #1. Please note that you can strip the first name from the full name field. Please use the following tag in your email alert to display first name:

    {name:first}
