on the form, even though i am 'CHECKING' the last box, i keep getting the attached screenshot message!
Also, there are 3 boxes on there for OPTIONS 1, 2 & 3... what are these 3 options relating to as there is no 'pop-up' explaining them?

This error message, judging by me indicates that you have some problem in internal dependancies on your form, let me replicate the form to chek this on my own.
regardign your second question they are from some other form or optional,
You can just delete options and leave just one check box for confirming that your users have:
I have read and understood all of the terms and conditions of this competition*

Please note that your last checkbox button is predefined as checked!
thats whay you see that error message.
Tryout this form that I changed a bit if it works and you like it
please replicate it
I have just removed two options bellow renamed first to I agree and unchecked the last check box

Just as datasphere has stated, the cause of the issue is the last checkbox option of your form:
However, the main cause is that you have not added any text to the option, so, it automatically gets selected by default. So, to resolve the issue simply add some text to the Option of the checkbox:
If you want to hide the option text, we can help you to find the CSS code to hide so that your form users cannot see it.
