received a "response for information request form" email

This morning I received an email from you guys stating that my name is "Bill" (which it is not) and that you received my information request form. The email also asks me what county in Pennsylvania do I live in. I never requested anything of you guys, and I don't live in PA. The email came from a "noreply@jotform.com" email address.

Please remove me from your email distribution list or whatever it is that is sending me emails.
