I embedded my form on a test webpage (url below) and it shows the form fine in chrome and IE - Also shows fine on my Galaxy android. But the form does not show on iphone unless i ommit some of the accompaying text on the same page.
I really need my forms to display on iphones.

UPDATE: the embed code is the left column of a table. The right column just has text in it. If i remove the content of the right column then the form displays. Also i notice that if i put th text back into the right column, the left and right column show up on the iphone for about half a second and then vanish!!
I also tried the iframe vesion and pasted tht onto my page, but no joy.
I also tried it on another iphone and stil lsame problem!

As far as I've observed, the problem is related with your tables in which the form is embedded. It seems your HTML codes are pretty messed up, it doesn't have html and body tags. I also found invalid codes at the bottom part of your page, some closing tags are redundant and some are missing. The form should display fine on iPhone as long as it is placed on clean table container. Sorry but the best way to have that fixed is to clean your HTML codes and review your tables. There are few nested tables on your page which is probably the caused.
If you checked the modified page right here: https://shots.jotform.com/elton/337255.html, you can see that it is viewable on iPhone. So, it is only a matter of structuring the tables.
