When clients fill the form on the ipad in our clinic, an error message comes up, when the form is filled on line via a desktop computer it seems to work fine.

Sorry but the form does not seem to be from Jotform. I assumed that is a prebuilt PHP form that works specifically on your site? Unfortunately, we cannot provide assistance about any 3rd party forms except from Jotform.
