results in 404 error

    When I click on "My Support Requests" (under the Support menu, showing a badge value of "2"), this is the page that tries to load:

    but it results in a 404 error: "Whoops! Unfortunately, the page you were looking for could not be found. It may be temporarily unavailable, moved or no longer exists. Please check your spelling and retry."

    Usually, this 404 error is displayed when there is no active session of your account. Please try logging into your HIPAA account and then try again to see the questions posted. Please note that the questions that are posted when you are logged into your Jotform account shall only be displayed.

    Thanks! Logging out and logging back in again did resolve the issue of not being able to see my two pending support requests.

    Oddly, amusingly, now I can't see *this* support request in my list of "My Support Requests". It's very strange, but (1) when I created this ticket, I was logged in (see first screenshot) but I couldn't see my previous support requests. Then, (2) after logging out and logging back in again as the SAME USER, my list of "My Support Requests" now shows but the list doesn't show THIS ticket (see second screenshot.)

    Looking more closely, it seems related to your HIPAA programming. I think I have only one account ( and it is a HIPAA account. But, even though I appear to have been logged in, I somehow created a ticket as if it was not a HIPAA account.

    Very strange... So, do I have two accounts tied to the same email, one HIPAA accout and one NOT-HIPAA account? If yes, how can I login to see the NOT-HIPAA account? In other words, if hypothetically I wanted to somehow login and view THIS ticket, how would I be able to view it again later?

    You are on a different link on your screenshots. If you want to see My Support Request, after logged in go to this link

    As for the account, you only have one account that is associated with this email which is under Patrick_Holmes username.
