How can I put "Others" using check box button?

    1. Create an other option to the checkboxes list
    2. Add a question to the form that says "Enter Other"
    3. Click on "Conditions" button on the "Setup & Share" toolbar
    4. Choose "Show / Hide a form field" option on the Conditions Wizard
    5. Create a condition:
    If "which option" field "equals to" "other"
    Then "show" "enter other" field
    6. Save the condition and save the form

    I have the same question as above, and though this solution succesfully implements the functionality, it doesn't achieve the look and feel I'm going for. Is there anyway to achieve the look of having a text box right after the check box and label as shown here?

    Yes, get the full form source code, then change it as you would like on your site.

    The form provided on JotForm is limited on the form builder. When you need to customize it further we provide source code for the form, you can get that, open it in your favorite HTML editor and change it. 

    Here is how you can get the form source code. Open the form on the Form Builder:

    1. Open "Setup & Embed" tab on toolbar,

    2. Click on "Embed Form"

    3. Click on "Source Code" and copy the code provided.

    Hodavidhara, I tinkered a bit with a clone of Aytekin's form to produce this result. Both fields were shrunk and using customized CSS the text box's top margin was set to 84 pixels and its label was hidden with the display:none; declaration.

    If you wish, you can clone and modify it to suit your needs. (In other words, clone my clone and modify my modifications! :) The "Recipient E-mail" value in the notification would need to be edited if you want to see how submissions from the form would look.
