I need to create as a medical form a multiple column table or list that allows a patient to fill in each row with the name of each medicine they are taking, the dosage amount and the frequency in which they are taking it. Would like the option for a patient to addition addtional rows, depending on the number of meds they're taking, but not to have the visible as blanks.
Heading and first row might be:
Name of medicine Strength/dosage how you take this medicine
Tylenol 325 mg 2 every 4 hours.
Then to have ability to add additional rows as necessary.
Like: Click here to enter an addtional medicine. Then a new row appears.
